The album is a concept record that tells the story of Nikki, a recovering drug addict who becomes involved with a revolutionary organization and is brainwashed into becoming an assassin. Produced by Peter Collins alongside the band, the album features guitarist Chris DeGarmo and vocalist Geoff Tate as primary songwriters, with the entire band contributing to the intricate progressive metal arrangements.

The album's narrative unfolds through tracks like "Revolution Calling," "Operation: Mindcrime," "Speak," and "Eyes of a Stranger," weaving together themes of political corruption, manipulation, and personal redemption. Sister Mary, a former prostitute turned nun who becomes Nikki's love interest, serves as a central figure in the story.
